<p class="wp-block-paragraph">I read a lot of GLP-1 research so you don&#8217;t have to, and 2026 has been a genuinely weird year for this drug class — real wins, a real flop, and a lawsuit pile that&#8217;s growing fast. Here&#8217;s the roundup, no spin.</p>



<p class="wp-block-paragraph">THE GOOD</p>



<p class="wp-block-paragraph">Heart failure: A large real-world study found starting a GLP-1 cut hospitalization or death from heart failure with preserved ejection fraction (HFpEF) by roughly 40% overall — 42% for semaglutide, 58% for tirzepatide, compared to a standard diabetes drug. That&#8217;s a big enough signal that the FDA added heart-failure outcome data to the prescribing information this year.</p>



<p class="wp-block-paragraph">Addiction: This one surprised even researchers. A Lancet-published randomized trial found semaglutide reduced heavy drinking days in people with alcohol use disorder. Separately, a large study of veterans found GLP-1 users had lower rates of alcohol, opioid, and nicotine use disorder. Nobody fully understands the mechanism yet, but the drugs seem to turn down cravings generally, not just food cravings.</p>



<p class="wp-block-paragraph">Suicide risk — actually good news for once: Remember the suicidal-ideation warning that got attached to these drugs a few years back? In January 2026, the FDA said it&#8217;s moving to remove that warning from semaglutide, tirzepatide, and liraglutide labels, based on a meta-analysis covering 91 trials and nearly 108,000 patients, plus a separate claims study comparing over 2 million people. No increased risk found. That&#8217;s a real reversal, and it doesn&#8217;t get nearly the coverage the original scare did.</p>



<p class="wp-block-paragraph">THE NOT-SO-GOOD</p>



<p class="wp-block-paragraph">Alzheimer&#8217;s —The Not the outcome we were hoping for: But that is why we do research.  <br>Novo Nordisk&#8217;s EVOKE and EVOKE+ trials tested oral semaglutide in people with early Alzheimer&#8217;s. It did not show a cognitive benefit versus placebo. This was a genuinely big, well-run trial, and it just didn&#8217;t pan out. If you saw hype about GLP-1s as a dementia treatment, file that under “not yet, maybe not ever.”</p>



<p class="wp-block-paragraph">Muscle loss: A preprint (meaning not yet peer-reviewed — treat it as early data) looked at roughly 670,000 GLP-1 users and found tirzepatide caused more lean muscle mass loss than semaglutide, with about 1 in 10 tirzepatide users hitting a pattern researchers called a “depletive metabotype” — losing more than 20% of body weight while losing more than 5% lean mass. <strong><em>This is exactly why every doctor keeps telling you to lift weights and eat your protein while you&#8217;re on these drugs. It&#8217;s not optional.</em></strong></p>



<p class="wp-block-paragraph">Litigation: As of August 2026, there are nearly 4,000 lawsuits consolidated against Novo Nordisk and Eli Lilly, up almost 200% since January 2025. Most allege gastroparesis (stomach paralysis), with smaller numbers citing bowel blockage or gallbladder problems. A separate, newer batch of cases focused on a vision condition called NAION is still working through the evidence stage — the vision risk itself is genuinely unresolved in the research, not just in the courtroom. Bellwether trials are expected late this year. No settlements yet.</p>



<p class="wp-block-paragraph">None of this is medical advice, and none of it means “don&#8217;t take these drugs” — I&#8217;m on tirzepatide myself and I&#8217;m not stopping over a preprint or a pending lawsuit. It does mean: talk to your doctor about your specific risk factors, get your labs and body composition checked periodically, and don&#8217;t let a scary headline (or an overly rosy one) make the decision for you instead of your doctor.</p>



<p class="wp-block-paragraph">I keep a running list of research updates like this for my own tracking — some of it&#8217;s in the book, and I&#8217;ll keep posting updates here as new studies land.</p>

<p class="has-medium-font-size wp-block-paragraph">If you’ve been following discussions about the Medicare GLP-1 Bridge program, you’ve probably noticed a pattern: a large number of people are reporting denials, especially those who have sleep apnea.</p>



<p class="has-medium-font-size wp-block-paragraph">This is not a coincidence. It is the result of a clear rule set by the Centers for Medicare &amp; Medicaid Services (CMS).</p>



<h1 class="wp-block-heading">The Official Rule</h1>



<p class="has-medium-font-size wp-block-paragraph">According to CMS, people who have a diagnosis of <strong>moderate-to-severe obstructive sleep apnea (OSA)</strong> are <strong>not eligible</strong> for the Medicare GLP-1 Bridge program.</p>



<p class="has-medium-font-size wp-block-paragraph">Here’s why:</p>



<ul class="wp-block-list">
<li class="has-medium-font-size">The Bridge program is only for people seeking GLP-1 medications (such as Zepbound KwikPen, Wegovy, or Foundayo) purely for weight management.</li>



<li class="has-medium-font-size">Moderate-to-severe sleep apnea is already considered a covered indication under regular Medicare Part D.</li>



<li class="has-medium-font-size">Zepbound has an FDA-approved indication specifically for moderate-to-severe obstructive sleep apnea in adults with obesity.</li>



<li class="has-medium-font-size">Because of this, CMS requires that anyone with this diagnosis go through their regular Part D plan instead of the Bridge program.</li>
</ul>



<p class="has-medium-font-size wp-block-paragraph">If a doctor submits a prior authorization to the Bridge program and the patient has an OSA diagnosis on record, the request will be denied.</p>



<h3 class="wp-block-heading has-medium-font-size">Other Common Reasons for Denial</h3>



<p class="has-medium-font-size wp-block-paragraph">While sleep apnea is one of the biggest triggers, it is not the only reason people are being turned down. Other frequent denial reasons include:</p>



<ul class="wp-block-list">
<li class="has-medium-font-size">Having Type 2 diabetes</li>



<li class="has-medium-font-size">Having noncirrhotic MASH (a form of fatty liver disease)</li>



<li class="has-medium-font-size">Already receiving a GLP-1 medication through a Part D plan</li>



<li class="has-medium-font-size">Not meeting the BMI requirements at the time GLP-1 therapy was started</li>



<li class="has-medium-font-size">Being prescribed a form of Zepbound that is not covered (only the KwikPen is allowed under the Bridge)</li>



<li class="has-medium-font-size">Incomplete prior authorization paperwork</li>
</ul>



<h3 class="wp-block-heading">What to Do If You Are Denied (Appeal Process)</h3>



<p class="has-medium-font-size wp-block-paragraph">This is an important point that many people do not realize:</p>



<p class="has-medium-font-size wp-block-paragraph"><strong>The Medicare GLP-1 Bridge program currently does not have a formal appeals process.</strong></p>



<p class="has-medium-font-size wp-block-paragraph">Unlike regular Medicare Part D denials (which have multiple levels of appeal), CMS has stated that there is no standard appeal pathway for Bridge program denials at this time.</p>



<p class="has-medium-font-size wp-block-paragraph">What you <em>can</em> do:</p>



<ol class="wp-block-list">
<li class="has-medium-font-size"><strong>Resubmit the prior authorization</strong><br>If the denial was caused by missing or incorrect information, your doctor can correct the paperwork and resubmit it.</li>



<li class="has-medium-font-size"><strong>Review the exact denial reason</strong><br>Both you and your doctor should receive a denial letter. Read it carefully. Many denials happen because the wrong pathway was used (for example, submitting to the Bridge when the patient has sleep apnea).</li>



<li class="has-medium-font-size"><strong>Switch pathways if needed</strong>
<ul class="wp-block-list">
<li>If you have moderate-to-severe sleep apnea → Try submitting through your regular Part D plan using the sleep apnea indication.</li>



<li>If you do not have a Part D-covered diagnosis → Work with your doctor to fix any documentation problems and resubmit to the Bridge.</li>
</ul>
</li>

<h4 class="wp-block-heading has-medium-font-size"><strong>Can New Information Help After a Past Sleep Apnea Diagnosis?</strong><br><br>Some people were diagnosed with obstructive sleep apnea (OSA) years ago but believe their condition has improved. In these cases, a few readers have asked whether updated information — such as overnight oxygen saturation (SpO2) readings from a fitness tracker — can make a difference.<br><br>While consumer devices like Garmin watches can track SpO2 trends during sleep and may show consistently normal oxygen levels, they are not considered a formal medical diagnosis. Medicare and the Bridge program generally require official clinical documentation, such as a recent sleep study or an updated evaluation from your doctor.</h4>



<p class="has-medium-font-size wp-block-paragraph"><br>If you previously had an OSA diagnosis but believe it is no longer accurate, the most useful step is to:</p>



<ul class="wp-block-list">
<li class="has-medium-font-size">Discuss your current symptoms (or lack of symptoms) with your doctor</li>



<li class="has-medium-font-size">Share any overnight SpO2 data you have collected</li>



<li class="has-medium-font-size">Ask whether a new sleep study or updated clinical note is appropriate</li>
</ul>



<p class="has-medium-font-size wp-block-paragraph">Your doctor can then decide whether the older OSA diagnosis should still be listed on a Bridge program application or whether the case should instead be submitted through regular Part D using a different indication.</p>



<p class="has-medium-font-size wp-block-paragraph"><strong>Final Thoughts:</strong></p>



<p class="has-medium-font-size wp-block-paragraph">The Medicare GLP-1 Bridge program has helped many seniors gain access to these medications at a lower cost. However, it was never designed for everyone. CMS intentionally excluded people with certain diagnoses, including moderate-to-severe sleep apnea, so those cases would stay under the normal Part D system.</p>



<p class="has-medium-font-size wp-block-paragraph">If you were denied, the first and most important step is to understand the exact reason. In many cases, the solution is not an “appeal” but using the correct coverage pathway or correcting the original submission.</p>

<p class="wp-block-paragraph">The Medicare GLP-1 Bridge program was created as a short-term demonstration by the Centers for Medicare &amp; Medicaid Services (CMS) to give eligible Medicare beneficiaries temporary access to certain GLP-1 weight-loss medications at a fixed $50 monthly cost. For years, Medicare was generally prohibited from covering drugs used primarily for weight loss. Growing rates of obesity and related health conditions among older adults created pressure for a limited program that could test broader access. The Bridge program officially began on July 1, 2026, and is currently scheduled to run through December 31, 2027.</p>

<p class="wp-block-paragraph"><br>Take advantage of your Medicare Advantage plan to maximize your benefits and improve your health on Medicare.<br><br>I spoke with a good friend who lives out of state in New Mexico about changing his lifestyle at 69 to help maximize his time left on this earth for his family and kids.<br><br>Like many of us at retirement, he just planted himself and let life take over.&nbsp; We stopped being in charge of your mental and physical health.<br><br>John is overweight and has bad knees and hips. His balance sucks. He drives for a ride-share service to have something to do, so he sits all day in his car.<br><br>In his two-story house, John has to go up and down stairs backward due to the lack of core stability and leg strength.&nbsp; He must use the handrail to climb stairs for stability.<br><br>He rides an indoor recumbent bike but hesitates to bike outdoors due to the fear of falling.&nbsp; He has zero flexibility in his back, neck, and hips.&nbsp; He cannot stand next to a bike and swing his leg over to mount it due to the lack of strength and loss of hip flexibility.<br><br>We had not talked in a few months when he mentioned he had lost 50 lbs. by eating one Mexican Food meal a day.<br><br>I asked him if he was serious about getting healthy. He confirmed he was, so I had him pick up a fitness tracker, a Garmin VivoActive 5, that also measures and tracks health metrics, including sleep, steps, HRV(heart rate variability), resting heart rate, SPO2(oxygen saturation—can be an insight into sleep apnea), respirations, and more. <a href="https://downhillslide.life/2024/08/09/fitness-trackers-can-help-improve-your-life-and-earn-you-cold-hard-cash/" data-type="link" data-id="https://downhillslide.life/2024/08/09/fitness-trackers-can-help-improve-your-life-and-earn-you-cold-hard-cash/">See my separate post on Fitness Trackers. </a><br></p>

<p class="wp-block-paragraph"><br>He received that watch, and I walked him through setting it up over the phone and using it to track exercise, such as an outdoor walk with GPS tracking and an indoor bike ride without GPS and data connection to his bike from the watch.<br><br>I reviewed the Garmin Connect app with him so he understands the “Garmin Vivoactive 5 System” and how to review his health metrics.<br><br>He has information on a Paleo lifestyle diet but has yet to commit.<br><br>Next, I gave him homework to review his Medicare Advantage plan online to familiarize him with his portal. <p class="wp-block-paragraph">So, John&#8217;s main issue stopping him from being more active is his lack of strength, flexibility, and balance. From a Medicare standpoint, he is having problems with some of his Activities of Daily Living.<br><br>His issues with “Activities of Daily Living” are on the Mobility Ambulatory front.<br><br>Here are the five main areas of ADL as explained by WebMD:</p>



<p class="wp-block-paragraph"><strong>Ambulating</strong>. This includes the ability to walk, sit, stand, lie down and get up, and climb up and down stairs, both inside and outside your home.</p>



<p class="wp-block-paragraph"><strong>Grooming</strong>. This includes all activities necessary to maintain personal hygiene, like brushing your teeth, bathing, shaving, and hair and nail care.</p>



<p class="wp-block-paragraph"><strong>Toileting</strong>. This includes the ability to control your bladder and bowels (continence), use the toilet safely, and clean yourself after use.</p>



<p class="wp-block-paragraph"><strong>Dressing</strong>. This involves the ability to dress yourself properly, including using buttons and zippers.</p>



<p class="wp-block-paragraph"><strong>Eating</strong>. This includes the ability to use cutlery and feed yourself.<br>(https://www.webmd.com/a-to-z-guides/what-are-activities-of-daily-living )<br><br><br>Now, we are working on getting him to review his Medicare Advantage plan to see what physical therapy benefits he has and getting him into physical therapy for strength and flexibility training.<br><br>With his Medicare plan, he has free gym access, but he needs help gaining the ability to work out independently.</p>



<p class="wp-block-paragraph">The goal for John is to get referred for 6-8 weeks in PT (paid for by Medicare &#8211; minus his CoPay amount per visit) to help him make safe, guided advances in his physical fitness, Strength, and Flexibility at Physical Therapy.&nbsp;<br><br>For those unfamiliar with Physical Therapy, it is based on what your PCP has written as the issue(s) to work on, e.g., balance, stair climbing strength, or flexibility to help you tie your shoes.&nbsp;<br><br>Your PCP can write up to 3 days a week for several weeks.&nbsp; Mine have always been 6-8 weeks, 2X a week.&nbsp;</p>



<p class="wp-block-paragraph">Every time I went, if there was still work to do on me (documented range of motion, strength,&nbsp; not back to normal), after the first 6-8 weeks, they could request that Medicare extend treatment, giving you almost double (an additional 6-8 weeks) if approved.<br><br>The physical therapist will take your history and evaluate you in many different ways. They may (depending on your issues) measure your range of motion(angles) of both arms/shoulders and hip flexion…. Then, compare this data to the general population of your age group. Show Hip Range of Motion from Spooner if possible.<br><br>Starting with stretches and strength exercises, concentrating on the target areas, they will add more reps/weight and new exercises to build strength and stability in the areas of need.&nbsp; Most of these exercises will be body weight, large rubber band, simple hand $10-$30 hand weight, or even a whole “can” of soup can be your weight.<br><br>Depending on your PT facility, they may have free weights and some machines, including bikes, steppers, and treadmills.&nbsp; Tell your PT what exercises you are doing or plan to do, both in the gym and outdoors, and they will make sure you have a plan down to sets/reps for a specific piece of equipment in your local gym that Medicare covers under your Renew Rewards Program.</p>



<p class="wp-block-paragraph">This is the road map for navigating this sometimes-daunting process.<br><br><strong>START HERE</strong>:<br><strong>1</strong>. Make an appointment with your PCP(Primary Care Physician) for your annual physical if you are due. Come in ready with a list of Daily Activities you are having issues with, such as walking, Stairs, Biking, Falling, and Balance.&nbsp;  Strength, Balance, and Flexibility are the major causes of injury in older adults, leading to falls.<br><br><strong>Easy Right?</strong><br><br>Activities of Daily Living are a big issue for Medicare and should be for each of us as we age.<br><br>Many PT programs can help with all those issues, but they require work on your part. <br>TINSTAAFL(Tin-Snaffle)&nbsp;= There Is No Such Thing As A Free Lunch<br><br>This is NOT “take a pill” for that. It&#8217;s hard work, pushing yourself, and probably some pain. You did not get out of shape overnight, so don’t expect overnight success.<br><br>It takes dedication to make health the most essential thing in one&#8217;s life. Without health, nothing else matters.<br><br>Make that appointment with your Primary Care physician today.&nbsp; After you hang it up, make that list of activities of daily living you are having issues with. <br>#MAHA<br><br><br></p>
